Pharmacology SBA Medium Sample · Q14 of 40

A 34-year-old patient is prescribed metronidazole for an acute dental infection. She mentions she is planning to attend a social event the following evening. Which of the following interactions should prompt the most urgent patient counselling?

ACo-prescription with paracetamol 500mg
BCo-prescription with ibuprofen 400mg
CConcurrent alcohol consumption
DCo-prescription with amoxicillin 500mg
ETaking the medication with food

✓
Correct answer: C — Concurrent alcohol consumption

Metronidazole inhibits aldehyde dehydrogenase, the enzyme that metabolises acetaldehyde (a by-product of alcohol). When alcohol is consumed concurrently, acetaldehyde accumulates — causing a disulfiram-like reaction characterised by severe flushing, nausea, vomiting, palpitations, and hypotension. Patients must be explicitly warned to avoid all alcohol during treatment and for 48 hours after the final dose.

Why the distractors score less: Paracetamol and ibuprofen have minor interactions with metronidazole in the short term. Amoxicillin is commonly co-prescribed for dental infections without significant concern. Taking metronidazole with food is actually recommended to reduce GI upset — not a risk.

What ORE candidates say

Used by overseas-qualified dentists at all stages of ORE Part 1 preparation.

★★★★★

"The progress dashboard changed how I revised. I could see immediately that I was weakest in Pharmacology and Materials, so I stopped wasting time on Anatomy where I was already scoring 85%. Targeted revision made all the difference — I passed Part 1 on my first attempt."

"I used two other question banks before this one. The difference is the explanations — they don't just tell you the right answer, they explain the clinical reasoning behind it. That's what the ORE Part 1 actually tests. My score went from 61% in week one to 83% by my exam date."

"The timed 40-question mock exams are what I needed most. Walking into Pearson VUE felt familiar because I'd already done it so many times in practice. I knew the format, I knew my pacing, and I wasn't anxious. Genuinely the best preparation tool I found for Part 1."
